Resident Services


Social Services

Our social workers will work with the resident and family before admission to complete a social history. Individual and family consultations as well as group services are used to help residents feel comfortable and secure. The admission coordinator/social services director, family and resident get together to discuss goals and plans. They closely monitor the resident’s progress towards independence. Many residents leave the facility to return to independent or modified lifestyles. Upon discharge, they can provide helpful advice such as care techniques, diet, medications and offers appropriate referrals to home health or community services.

Dining

Meal times are meant to be enjoyed with family and friends. Our dining room is designed to promote socialization among residents while creating an unparalleled first class dining experience. All meals are prepared by our trained chefs under the direction of a registered dietitian. We also offer a private family dining room for special occasions, which can be reserved by contacting our administrative offices.

Activity Programs

Regular active participation in our activity programs can be both stimulating and gratifying for our residents and families. Each day residents are offered a diverse calendar of events tailored to their individual needs. While they are free to participate in as little or as much as they like, active involvement is encouraged. Family and friends are also encouraged to participate as well. Continental Nursing and Rehabilitation Center offers structured programs that provide a home like atmosphere where residents are treated with sensitivity, dignity and respect. We cater to our residents through physical and mental stimulation.

Meals

Dining at Continental is an opportunity to socialize among friend and enjoy a satisfying meal served by our staff. Holidays and celebrations are special occasions greeted with colorful decorations, cheerful festivities and the welcome participation of family and friends. Because we take great pride in our choice of foods, residents who wish to have a selection other the main entrée are given a list of alternate dishes. Everyone receives three balanced, nutritious meals daily, and meals are always prepared according to each resident’s dietary requirements.

Personal Items

We encourage families to bring in personal items such as photos, clocks, radios, even a favorite chair where space permits. Each resident’s room space includes a bed, arm chair, chest of drawers and nightstand. Television cable hook-up and telephone jack are also provided at no extra charge.
